The CareSpace portal will allow give patients and their caregivers the ability to see their health information and communicate with their providers anytime, anywhere. SeeYourChart is replaced with CareSpace with better functionality and information. It is a platform developed from the ground up that optimizes on stability, security, and agility. CareSpace has modern patient-user interaction that is intuitive to use and compatible with desktop, laptop, tablet and mobile. By being mobile compatible, patients can access their information on the go.
Through CareSpace patients or their authorized caregiver can:
Access their health information anytime, anywhere.
Communicate securely with the Tri County care team.
Share access to their portal with family and caregivers.
Securely send health information to providers outside of Tri County, like their primary care providers.
View next appointment. This allows patients to view all upcoming, scheduled appointments with basic detail about appointments.
View Lab results on the “Health: page of CareSpace once results are approved by the Clinician.
CareSpace will provide patients specific document types that are helpful for their care. Patients can view and download their signed and “finalized” Care Plans. Patient Education materials from the document section.
